Why Mentoring Works

One point of contact for a complicated system

Recovery often means juggling therapists, doctors, programs, and paperwork. Case management puts one experienced person in charge of the moving parts, so you can focus on healing instead of logistics.

One point of contact

A single person coordinating providers, appointments, and logistics, so you do not have to.

Nothing falls through the cracks

Careful tracking of the moving parts that recovery depends on.

An advocate in your corner

Someone who knows the system and helps you navigate it with confidence.

Is This Familiar

When case management helps

  • Recovery involves several providers and moving parts
  • Appointments, referrals, and follow-ups keep slipping
  • A family is overwhelmed coordinating a loved one care
  • You need help navigating treatment and resources
  • You want one person tracking the whole picture
  • Insurance and logistics feel like a second job
  • A transition between levels of care is coming
  • You want continuity, not a handoff and silence

Recovery is hard enough without managing the machinery around it. We keep the pieces moving so you can focus on healing.

Our Support

What recovery case management includes

Assessment icon

Assessment & Planning

A thorough look at where things stand and a clear, coordinated plan for what comes next. We assess the full picture, clinical, practical, and personal, then build a roadmap that turns a scattered set of needs into an organized, achievable path forward.

Coordination & Scheduling

One point of contact who keeps the appointments, providers, and moving parts in sync. Instead of juggling a dozen phone numbers and calendars alone, you have someone making sure everything connects and nothing falls through the cracks.

Aftercare Support ›

Communication icon

Provider Communication

Steady communication between the therapists, doctors, and programs involved in your care. We keep everyone informed and aligned, so your team actually works as a team and you are not the one relaying messages between people who should be talking directly.

Family Support ›

Logistics icon

Resources & Logistics

Practical help navigating the resources, paperwork, and logistics that recovery so often requires. From treatment options to benefits to everyday arrangements, we handle the legwork that would otherwise become one more reason to give up.

Recovery Resources ›

Transitions icon

Transitions of Care

Careful management of the handoffs between levels of care, where people most often get lost. We make sure the move from one program or provider to the next is planned and supported, so momentum carries through instead of stalling in the gap.

ONE COORDINATED PLAN

Recovery is hard enough without managing it alone

Between providers, appointments, paperwork, and transitions, coordinating care can become a full-time job at the exact moment you have the least capacity for it. Case management puts one organized, knowledgeable person in charge of the moving parts, so you can put your energy into recovery instead of logistics.

How It Works

How support begins

01

A private conversation

Confidential and without pressure. We listen first and help you name what is really happening.

02

A support plan

Built around your patterns, your triggers, and the shape of your week.

03

Real-world support

Consistent presence between appointments and after treatment, where recovery is actually tested.

04

Coordinated care

We work alongside your clinical team when one exists, or help you build one through Heights Behavioral Health.

Let us handle the moving parts

A private, judgment-free conversation is the first step. We will help you find footing, whatever the last few months have looked like.

Where Mentoring Fits

Non-clinical support that works with your care

Heights Mentoring is a private, non-clinical recovery support practice. We do not diagnose or provide therapy, psychiatry, or medical care. When those are the right next step, we coordinate with licensed providers and our sister clinical practice, Heights Behavioral Health, so nothing falls through the cracks.

WHAT WE ACTUALLY COORDINATE

One Person Holding the Whole Picture

Families rarely fail at caring. They fail at logistics, because nobody was ever given the job of holding all of it at once.

  • Psychiatrist

Every arrow is a conversation somebody has to have. Case management means it is us, not you.

Providers and Appointments

Therapists, psychiatrists, primary care, testing, and programs, scheduled and confirmed rather than hoped for. One calendar, one point of contact.

Records, Releases, and Admissions

Consents, records requests, and admissions paperwork handled properly, so care is not delayed by a form nobody knew was missing.

Family Communication

Regular, honest updates within the limits your loved one agrees to, so families stop guessing and start supporting the actual plan.

Case management often runs alongside recovery coaching and family support.

Questions

Case management questions, answered

Case management is the administrative and logistical work that families usually end up doing badly at midnight. It covers scheduling and confirming appointments across therapists, psychiatrists, primary care and programs, handling consents and records requests, managing admissions paperwork, tracking medication follow-up, and giving the family one honest point of contact for updates. The goal is that nobody is guessing what anyone else is doing.

Coaching works on the person and their week. Case management works on the system around them. A coach helps someone get through Thursday; a case manager makes sure the psychiatrist, the therapist, the program and the family are all working from the same plan and that no referral is sitting in a drawer. Most families need both, and they are often delivered together with recovery coaching.

No. If a treatment program has a discharge planner or a hospital has a social worker, we work with them rather than around them. Where we add value is after discharge, when institutional case management ends and the family is suddenly holding everything alone. That handoff is the gap this service exists to cover.

No. This is non-clinical care coordination. We do not diagnose, treat, or make clinical decisions. We make sure the people who do are talking to each other, that appointments happen, and that paperwork is not the reason care stalls. Clinical oversight of our standards comes from licensed clinicians at Heights Behavioral Health.

Exactly what was agreed in writing at the start, and nothing more. Adults set the terms of what is shared. Safety concerns always reach the family immediately. Everything else runs to the boundaries the client consented to, which is what makes the relationship usable in the first place.

Yes. If there is no clinical team in place, we help you build one, including assessment through Heights Behavioral Health or a referral elsewhere if that is the better fit. Our levels of care guide explains how detox, residential, PHP, IOP and outpatient fit together so you can follow the recommendation rather than take it on faith.

Usually within a day or two, and often the same day when there is a discharge or an admission in motion. Call (713) 337-5063 and tell us the timeline you are working with.
