The Gap We Fill

The support outpatient care cannot provide on its own

Outpatient programs do vital clinical work, but clients go home to real life between sessions. That is where relapse risk lives, and where we come in, with structure, accountability, and presence that make outpatient care hold.

Structure between program hours

Real-world support in the many hours outpatient care cannot cover.

Accountability that protects progress

Check-ins and follow-through that keep clinical gains from slipping.

A bridge as intensity steps down

Steady continuity as clients move from PHP to IOP to outpatient and beyond.

When It Helps

When outpatient care needs a partner

  • A client is in PHP or IOP and going home each night
  • The hours between sessions feel unsupported
  • Progress in program does not translate to home
  • Old triggers wait in daily life
  • A step-down in care is coming
  • A weekly session leaves gaps
  • Accountability is needed to protect gains
  • The family wants continuity, not a cliff

Outpatient care and mentoring are stronger together. We reinforce the clinical work out in the real world.

How We Help

How we support outpatient clients

Between sessions icon

Between-Session Support

Steady support in the many hours your program does not cover. IOP and PHP do vital work in session, but recovery is tested in the evenings, weekends, and gaps between, and that is exactly where we keep you supported and on track.

Accountability icon

Accountability & Check-Ins

Regular, honest check-ins that keep your program’s work from fading between visits. We reinforce what you are learning in treatment and hold you accountable to it in daily life, so the progress actually compounds.

Sober Coaching ›

Step down icon

Step-Down Continuity

Continuous support as you step down from higher levels of care. The move from PHP to IOP to outpatient is where many people lose momentum, so we keep the structure steady while the clinical intensity decreases.

Aftercare Support ›

Family icon

Family Support

Guidance for families supporting a loved one through outpatient care. We help the household understand the process and hold helpful boundaries, so home reinforces the program instead of undermining it.

Family Support ›

Coordination icon

Coordination With Your Program

Close coordination with your treatment team, not around it. We complement your clinicians and communicate with them, so your outpatient program and your mentoring pull in the same direction.

Case Management ›

Relapse prevention icon

Relapse Prevention

A real-world relapse-prevention plan for the exposed hours outside the clinic. We rehearse your high-risk moments and build responses you can use immediately, so a hard evening does not undo a good week in program.

THE HOURS PROGRAM DOES NOT COVER

Outpatient works better when someone has the other twenty hours

IOP and PHP do essential clinical work, but you are only in session a few hours a week, and recovery is won or lost in all the hours in between. Non-clinical support wraps around your program, keeping the accountability and structure alive at night, on weekends, and in the moments your clinicians cannot be there.

How It Works

How support begins

01

A private conversation

Confidential and without pressure. We listen first and help you name what is really happening.

02

A support plan

Built around your patterns, your triggers, and the shape of your week.

03

Real-world support

Consistent presence between appointments and after treatment, where recovery is actually tested.

04

Coordinated care

We work alongside your clinical team when one exists, or help you build one through Heights Behavioral Health.

Make outpatient care stick

A private, judgment-free conversation is the first step. We will help you find footing, whatever the last few months have looked like.

Where Mentoring Fits

Non-clinical support that works with your care

Heights Mentoring is a private, non-clinical recovery support practice. We do not diagnose or provide therapy, psychiatry, or medical care. When those are the right next step, we coordinate with licensed providers and our sister clinical practice, Heights Behavioral Health, so nothing falls through the cracks.

THE HOURS NOBODY BILLS FOR

Outpatient Care Ends at Five. Recovery Does Not.

A strong outpatient program might cover nine hours a week. The other one hundred and fifty-nine are where the plan is either kept or quietly abandoned.

Evenings, Weekends, and Holidays

The hardest hours are rarely business hours. We are present in the ones that actually carry risk, including the ones your program cannot staff.

Getting There, Every Time

Missed sessions are the first sign of drift. Transport, reminders, and honest accountability keep attendance from becoming the first thing to slip.

Talking to Your Program

With your permission we keep your clinical team informed, so what happens at home on Thursday is known in session on Friday rather than months later.

The Road Home

The road from treatment to a life of your own

Clinical care moves in steps, and each one matters. But the most dangerous stretch is the one nobody staffs: the space between the last program and a life that holds on its own. That is exactly where we work.

01

Detox

Medical stabilization, safely managed

02

Inpatient

Residential care, around the clock

03

PHP

Full-day treatment, evenings at home

04

IOP

Treatment that shares the week with real life

Questions

Outpatient support questions, answered

Outpatient care, ASAM Level 1, is usually one to eight clinical hours a week: individual therapy, group work, and psychiatric review where medication is involved. It is the right level for people who are medically stable and have a workable living situation. What it does not cover, and does not claim to, is the other one hundred and sixty hours in the week.

Because outpatient is designed to treat, not to supervise. Someone can be fully engaged in session on Tuesday and completely unsupported at nine on Friday evening. Relapse rarely happens in the therapy room. It happens in unstructured time, in the old environment, with the old people, at the hours no clinician is rostered.

We take the clinical plan and make it operational: getting to sessions, holding the between-session assignments, covering evenings and weekends, and reporting back to the clinical team with written consent so nothing is discovered weeks late. We never duplicate or contradict the clinical work. Formal coordination across multiple providers runs through case management.

No, and it should not be sold as one. Mentoring is non-clinical support. If somebody needs therapy, medication management or a higher level of care, that is a clinical need and we route it to a licensed provider rather than absorbing it.

Common signals: repeated missed sessions, use continuing despite engagement, an unsafe or triggering home environment, withdrawal risk, or an untreated psychiatric condition underneath. Our levels of care guide walks through how those judgments are made and what the next step usually is.

Yes. If you do not have one, we help you get assessed and placed, through Heights Behavioral Health or elsewhere if that fits better. We would rather refer you accurately than keep you.

With written consent, yes. We join treatment team calls, take the discharge and continuing-care recommendations seriously, and give the program honest feedback about how the plan is holding at home.
