Understanding Levels of Care
The ASAM Criteria is the national clinical standard for matching a person to the right level of addiction and mental health care, from early intervention through intensive treatment. The continuum is commonly described on a scale from 0.5 to 4, where higher numbers indicate more intensive, medically involved services. Within each main level, decimal specifiers describe more granular differences in intensity, staffing, and structure. Recovery mentoring and coaching is a non-clinical recovery support service that complements every level. For clinical levels of care in Houston, including outpatient, IOP, and PHP, we work alongside our sister practice, Heights Behavioral Health.

A licensed clinician uses a multidimensional, biopsychosocial assessment based on the ASAM Criteria, weighing withdrawal risk, medical and mental health needs, readiness to change, relapse risk, and living environment, then matches you to the least restrictive level that keeps you safe. The ASAM Continuum
LEVEL 0.5
EARLY INTERVENTION
Level 0.5 refers to services for individuals at risk of developing a substance use disorder but who do not yet meet diagnostic criteria. These services might include screening, brief intervention, and referral to treatment delivered in primary care, workplaces, schools, or community settings.
LEVEL 1
OUTPATIENT SERVICES
Level 1 is the least intensive formal treatment level, focused on flexible, non-residential care. Outpatient programs generally provide fewer than 9 hours per week of structured services for adults, often 1 to 3 sessions weekly.
LEVEL 2
HIGH-INTENSITY OUTPATIENT
Level 2 covers non-residential but significantly more structured and time-intensive programs than standard outpatient. It includes intensive outpatient (IOP) and partial hospitalization (PHP), described in the cards below.
LEVEL 2.1
INTENSIVE OUTPATIENT PROGRAM (IOP)
At least 9 hours of structured clinical services per week for adults, often grouped into multiple half-day sessions across the week. Care targets both substance use and co-occurring issues (mental health, relapse risk, unstable housing) using counseling and psychoeducation.
LEVEL 2.5
PARTIAL HOSPITALIZATION (PHP)
About 20 or more hours of weekly services, often delivered as full-day or near-full-day programming several days per week. This is used when individuals need robust daily structure and therapeutic contact but still can live in the community.
LEVEL 2.7
MEDICALLY MANAGED INTENSIVE OUTPATIENT
Programs with more active medical oversight, such as frequent physician or nurse contact and medication management for complicated withdrawal or comorbidities, while still remaining non-residential.
LEVEL 3
RESIDENTIAL | INPATIENT (NON-HOSPITAL)
Level 3 includes 24-hour structured treatment in a residential environment, ranging from low-intensity therapeutic communities to medically monitored programs that approach hospital-level care. Key sublevels are described in the cards below.
LEVEL 3.1
CLINICALLY MANAGED LOW-INTENSITY RESIDENTIAL
Focuses on community reintegration, often for individuals needing a sober, structured living environment with at least 9 hours of clinical services weekly. There is strong emphasis on psychoeducation and life skills while medical services are less intensive and usually provided off-site or on a consult basis.
LEVEL 3.3 & 3.5
CLINICALLY MANAGED MEDIUM & HIGH-INTENSITY RESIDENTIAL
Provide more intensive clinical interventions for people with cognitive, behavioral, or environmental barriers to recovery who need close supervision and daily therapeutic programming. Level 3.5 is often used for individuals with significant imminent risk of harm, repeated relapses, or severe functional impairment who still do not require continuous medical monitoring.
LEVEL 3.7
MEDICALLY MONITORED INTENSIVE INPATIENT | RESIDENTIAL
Offers 24-hour nursing care with physician oversight for individuals with severe withdrawal risk, serious biomedical conditions, or complex psychiatric needs. Many detox units and higher-acuity residential centers operate at this level.
LEVEL 4
MEDICALLY MANAGED INPATIENT
Level 4 is the most intensive care, typically hospital-based, for individuals requiring fully medically managed detox or stabilization. These programs provide 24-hour nursing care and daily physician care for severe or unstable intoxication, high-risk withdrawal, serious biomedical complications, or acute psychiatric and cognitive conditions.
